Care management is a professional service that helps individuals and families coordinate, navigate, and manage healthcare and support needs. This includes assessment, planning, advocacy, and ongoing oversight to ensure care is appropriate, effective, and aligned with personal values. 


Qualitative Care Management is a professional care management and advocacy service — not a home health agency. We do not provide hands-on medical care or personal care services.

Instead, we coordinate and oversee care, advocate for our clients, and guide families through complex care decisions with clarity and confidence. We frequently work alongside home health agencies, caregivers, and medical providers to ensure care is appropriate, well-coordinated, and aligned with the client’s needs.

In addition, we offer Professional Support Visits (non-medical respite support), which provide safety monitoring, companionship, and oversight in the home. These visits are designed to give family caregivers time to rest while maintaining professional awareness of the client’s well-being.   This concierge model provides families with both expert care oversight and trusted in-home support — delivering peace of mind when it matters most. 


Professional Support Visits are non-medical, oversight-focused visits that provide safety monitoring, companionship, and structured support in the home. These visits are designed to give families peace of mind while ensuring clients remain safe and supported. 


 No. Qualitative Care Management provides non-medical support, supervision, and care oversight. We do not provide licensed home health or personal care services. When hands-on care is needed, we help families coordinate with appropriate licensed providers. 


Traditional home care agencies focus primarily on hands-on assistance. QCM Support Visits provide a more professional, oversight-driven approach — combining companionship, safety monitoring, and experienced care observation within a boutique care management model. 


Yes. Many families use Support Visits as a structured respite solution, allowing primary caregivers time to rest, attend appointments, or manage other responsibilities while their loved one remains safely supported.

Visits are typically scheduled in 2- to 4-hour blocks, depending on family needs and client safety considerations. 


Yes. Support & Respite Visits are private-pay concierge services. 


 No. We are not a medical provider. We collaborate with medical professionals and help ensure care plans are understood, followed, and adjusted as needed. 


We primarily serve individuals and families in Central Virginia. Some services may be available remotely depending on the situation. 


Yes. Trauma-informed care is foundational to our practice. We recognize that illness, aging, loss of independence, and caregiving can be deeply emotional and, at times, traumatic. We approach every situation with empathy, respect, and sensitivity. 


Care management can be short-term or ongoing, depending on needs. Some clients require support during a specific transition, while others benefit from long-term coordination and advocacy. 


Absolutely. Many of our clients are adult children managing care from a distance. We act as trusted eyes, ears, and advocates on the ground.
