Requirements include:

•  Certification or license in a healthcare profession.  
• A minimum of 1 year of experience in a healthcare facility or agency.

• Possess the ability to collaborate effectively with interdisciplinary healthcare teams.,

• Must have a valid Virginia driver's license and adequate vehicle insurance.

Responsibilities include but are not limited to:

• Ensure the overall well-being and safety of the clients.
• Provide routine hygienic care.
• Assist with bathing, grooming, and toileting.
• Take and record vital signs.
• Measure and record height and weight, intake and output.
• Assist with eating and hydration.
• Meet guests’ mental health and social service needs.
• Ensure all activities of daily living are maintained.
• Provide transportation or travel to appointments as necessary with clients.
• Provide light housekeeping to maintain the cleanliness of the entire living quarters.
• Collaborate with interdisciplinary care team members.
•  Collaborate with emergency services and healthcare providers to address urgent situations.
• Inform care team members of emergencies and observations of potential health decline.
• Documentation Management.
• Participate in educational seminars to promote trauma-informed awareness.
• Duties will vary based on the client's needs.

We are hiring part-time human services professionals.  The desired candidates will possess:

• Minimum of a bachelor's degree in a human services field.
• Knowledge in trauma-based practices.  
• Have compassion and empathy.
• Be observant, patient, and concerned with the overall well-being of people.  
• Have strong communication and interpersonal skills.
• Possess the ability to collaborate effectively with interdisciplinary healthcare teams.
• Express empathy and sensitivity to the unique needs and challenges of elderly individuals.

Responsibilities include:

• Conduct comprehensive assessments and treatment planning. 
• Collaborate with interdisciplinary care team members.
• Provide care while utilizing evidence-based practices.
•  Assess health needs, identifying symptoms of anxiety, depression, grief, and other mental health issues. 
•  Construct personalized treatment plans based on assessment findings and collaborate with patients, families, and facility staff in their implementation. 
•  Monitor progress and adjust treatment plans as needed to obtain overall well-being. 
•  Collaborate with emergency services and healthcare providers to address urgent situations.
•   Develop and distribute informational materials for team members.
•  Conduct educational sessions for team members.
•  Generate regular reports on patient progress, treatment outcomes, and other relevant metrics.
