During the initial assessment, the case manager realized that the medication-tracking system we were using was inadequate and designed an electronic format that we still print weekly. She makes graphs of health data regarding my mother for her to take with her to doctor appointments. The medication log was so helpful to each of us to ensure Mom received each of her medications when it was time."
